| dc.description.abstract | Este proyecto aborda la brecha en la gestión del riesgo en Atención Primaria en Salud dentro de los servicios hospitalarios de una Empresa Social del Estado de mediana complejidad en Boyacá, Colombia. El Hospital Regional de Duitama documenta apenas el 12% de los egresos con actividades preventivas según la Resolución 3280 de 2018, generando glosas contractuales de entre $2M y $8M COP por auditoría y ausencia de seguimiento para la población más vulnerable, especialmente pacientes del régimen subsidiado con enfermedades crónicas sin control. El proyecto se desarrolló en el servicio de urgencias del Hospital Regional de Duitama, Empresa Social del Estado de mediana complejidad, referente de tres provincias de Boyacá. Los usuarios operadores son médicos especialistas en medicina familiar y los beneficiarios directos son pacientes del régimen subsidiado, con predominio de población vulnerable con enfermedades crónicas sin control, residente en zonas rurales y semirurales del municipio. Se diseñó la plataforma web Medicina Familiar Integral Nodo APS, que integra el registro sistemático de actividades preventivas al egreso hospitalario en menos de dos minutos, sin integración con el HIS institucional, con alertas automáticas al equipo de Promoción y Prevención y dashboard gerencial en tiempo real. Se aplicó un enfoque híbrido de Design Thinking, Lean Startup y Design Sprint: diez entrevistas semiestructuradas, observación de dos turnos en urgencias y pruebas de usabilidad con usuarios reales. El prototipo V2 registró cero rechazos al concepto, tiempo de registro de 1:48 a 2:30 minutos, cinco mejoras incorporadas del campo y decisión estratégica de ajustar. Se proyecta alcanzar el 70% de egresos con registro APS al mes tres. La solución es viable técnica, clínica y financieramente: ROI de 8x 15x, VPN de $312M COP, TIR del 187% y potencial de escalamiento a 45 ESE en Boyacá. | spa |
| dc.description.abstract | This project addresses the gap in risk management in primary health care within the hospital services of a medium-complexity state-owned social enterprise in Boyacá, Colombia. The Duitama Regional Hospital records preventive care activities for only 12% of discharges, as required by Resolution 3280 of 2018, resulting in contractual penalties ranging from $2M to $8M COP per audit and a lack of follow-up for the most vulnerable population, particularly patients in the subsidized health plan with uncontrolled chronic diseases. The project was carried out in the emergency department of the Duitama Regional Hospital, a medium-complexity State-Owned Social Enterprise that serves as a referral center for three provinces in Boyacá. The end-users are physicians specializing in family medicine, and the direct beneficiaries are patients in the subsidized health plan, predominantly vulnerable individuals with uncontrolled chronic diseases residing in rural and semi-rural areas of the municipality. The “Medicina Familiar Integral Nodo APS” web platform was designed to systematically record preventive activities upon hospital discharge in less than two minutes, without integration with the institutional HIS, featuring automatic alerts to the Health Promotion and Prevention team and a real-time management dashboard. A hybrid approach combining Design Thinking, Lean Startup, and Design Sprint was applied: ten semi-structured interviews, observation of two shifts in the emergency department, and usability testing with real users. The V2 prototype recorded zero rejections of the concept, a registration time ranging from 1:48 to 2:30 minutes, five improvements incorporated from the field, and a strategic decision to make adjustments. The project is projected to achieve 70% of discharges with APS registration by month three. The solution is technically, clinically, and financially viable: ROI of 8x–15x, NPV of $312 million COP, IRR of 187%, and potential for scaling to 45 ESEs in Boyacá. | eng |
