| dc.description.abstract | La resistencia antimicrobiana se ha consolidado como uno de los mayores desafíos para la salud pública a nivel mundial. Este fenómeno ocurre cuando los microorganismos desarrollan mecanismos que les permiten sobrevivir a la acción de los antimicrobianos utilizados para combatirlos (OMS, 2020). reduciendo progresivamente la eficacia de los tratamientos disponibles. Como consecuencia, infecciones que antes podían controlarse de manera efectiva se han convertido en un problema cada vez más complejo para los sistemas de salud. Durante las últimas décadas, diferentes factores han contribuido al aumento de la resistencia bacteriana. Entre ellos se encuentran el uso inadecuado de antibióticos, la automedicación, el incumplimiento de los tratamientos prescritos y el incremento de infecciones asociadas a la atención en salud. (O'Neill, 2016; Prestinaci et al., 2015). Estas situaciones han sido asociadas en la literatura con la aparición y propagación de microorganismos resistentes , limitando las alternativas terapéuticas disponibles y aumentando el riesgo de complicaciones clínicas. La evidencia reportada por estudios científicos y sistemas de vigilancia epidemiológica muestra cambios importantes en los patrones de sensibilidad bacteriana y en la distribución de microorganismos resistentes tanto en entornos hospitalarios como comunitarios. En Colombia, al igual que en otros países, esta problemática genera preocupación debido a las implicaciones que diversos estudios han descrito sobre la morbimortalidad el aumento de los costos de atención y la necesidad de recurrir a tratamientos cada vez más complejos (Antimicrobial Resistance Collaborators, 2022). Con el propósito de promover un uso más adecuado de los antibióticos y disminuir la presión selectiva sobre las bacterias, la Organización Mundial de la Salud (OMS) implementó en 2017 la clasificación AWaRe (Access, Watch y Reserve) (OMS, 2023). Esta estrategia busca orientar la prescripción de antimicrobianos de acuerdo con su importancia terapéutica y su potencial para generar resistencia. De forma complementaria, los Programas de Optimización de Antimicrobianos (PROA) han fortalecido las acciones de vigilancia y control del consumo de estos medicamentos en distintos sistemas de salud. (Sharma et al., 2022; Li et al., 2024; Liu et al., 2025). En este contexto, resulta pertinente analizar las tendencias de consumo de antimicrobianos en Colombia desde la implementación de la clasificación AWaRe y compararlas con las dinámicas observadas a nivel internacional. Comprender estos comportamientos permitirá identificar fortalezas, brechas y posibles riesgos relacionados con el uso de antibióticos, además de aportar evidencia útil para fortalecer las estrategias orientadas a prevenir la resistencia bacteriana y promover un uso racional de los antimicrobianos en el país. | spa |
| dc.description.abstract | Antimicrobial resistance has become one of the greatest challenges to global public health. This phenomenon occurs when microorganisms develop mechanisms that allow them to survive the action of antimicrobials used to combat them (WHO, 2020), progressively reducing the effectiveness of available treatments. As a result, infections that could previously be effectively controlled have become an increasingly complex problem for healthcare systems. Over the past decades, various factors have contributed to the increase in bacterial resistance. These include the inappropriate use of antibiotics, self-medication, non-adherence to prescribed treatments, and the rise in healthcare-associated infections (O'Neill, 2016; Prestinaci et al., 2015). These situations have been associated in the literature with the emergence and spread of resistant microorganisms, limiting available therapeutic options and increasing the risk of clinical complications. Evidence reported by scientific studies and epidemiological surveillance systems shows significant changes in bacterial susceptibility patterns and in the distribution of resistant microorganisms in both hospital and community settings. In Colombia, as in other countries, this issue raises concern due to the implications described in various studies regarding morbidity and mortality, increased healthcare costs, and the need to resort to increasingly complex treatments (Antimicrobial Resistance Collaborators, 2022). With the aim of promoting more appropriate antibiotic use and reducing selective pressure on bacteria, the World Health Organization (WHO) implemented the AWaRe classification (Access, Watch, and Reserve) in 2017 (WHO, 2023). This strategy seeks to guide antimicrobial prescribing according to therapeutic importance and the potential to promote resistance. In addition, Antimicrobial Stewardship Programs (ASP) have strengthened surveillance and control measures for the consumption of these medications in different healthcare systems (Sharma et al., 2022; Li et al., 2024; Liu et al., 2025). In this context, it is relevant to analyze antimicrobial consumption trends in Colombia since the implementation of the AWaRe classification and compare them with trends observed internationally. Understanding these patterns will make it possible to identify strengths, gaps, and potential risks associated with antibiotic use, as well as provide useful evidence to strengthen strategies aimed at preventing bacterial resistance and promoting the rational use of antimicrobials in the country. | eng |
